There is no personal reason reported, so the most obvious reason is, he trains worse than his teammates.

3 seperate coaches confirmed that Ansu doesn't train like he should, Flick hinted it several times. I don't know if it's because he's lazy or he just can't do it but it doesn't even matter, he's not good enough for this level. The club wanted that he leaves but he refused and now he's making a fuss even though they told him he won't play but the salary keeps him here. He's getting worse that Umtiti and similar leaches did...

It was after the two surgeries/rehab attempts that failed.

He's had incredibly bad luck. It was reported that the club's rehab process botched it and not the surgeries itself.

Which is why Mendes and Ansu did the final surgery with someone not associated with the club and planned the rehab afterwards with someone else.

Damage was already done though.
